Midwest Palliative & Hospice CareCenter is a nonprofit, community-based healthcare organization that provides medical care as well as supportive services to people where they live, from the time of diagnosis of a short-term, chronic or life-threatening condition, through and after treatment.
Programs include:
- Home Health Services (short-term, in-home healthcare)
- Medical Home Visits (short-term or long-term house calls for the elderly)
- Palliative Care Consultation (consults for pain and symptom management)
- HomeCare Assistants (private-duty caregiving services - especially helpful for senior citizens aging at home)
- Hospice
- Grief Support (available to CareCenter hospice families as well as the greater community)
CareCenter programs are covered by Medicare and most other insurances, with the exception of private-duty caregiving. Although the services provided by HomeCare Assistants are not covered by Medicare or other medical insurance, they are often covered under long-term-care policies, and private payment is also accepted. Charity care and financial assistance are available for those who qualify.
Midwest CareCenter's service area includes Lake County and most of Northern Cook County. In addition, hospice and grief support services are provided in some of the southeast suburbs of Cook County.
